Instructions for Completing Pro Se Forms

Complete the appropriate parts of the form including the attachment as it applies to your case. The link for the attachment is below.
Sign the completed forms.
Present the document to the Child Support Agency if you have a case with that office or if anyone is receiving or applying for Public Assistance (Medical Assistance, Child Care, Food Stamps, W-2)
Bring in the document for the Judge's signature.
After the document is signed by the Judge, make three (3) copies.
Bring the original document and the three (3) copies to the Clerk of Courts Office for Filing.

The original document is kept by the Clerk of Courts
The copies are for:

Respondent
Child Support Agency

Divorce or Legal Separation and Marital Settlement - A completed Marital Settlement form must accompany the Pro Se Findings of Fact form when going through a Pro Se Divorce, otherwise child support will not be addressed in your order!!

Stipulation and Order to Amend Judgment for Support/Maintenance/Custody/Placement

When parties are in agreement to modify their existing orders, they can complete and file the Stipulation and Order to Amend Judgment for Support/Maintenance/Custody/Placement with the court. Once approved by the OCSE and Family Court Commissioner, the order will be modified without the need for a hearing.

If the parties are not able to agree on a modification of Support, Maintenance, Custody and/or Placement, either party can opt to file Notice of Motion and Motion to Change or Order to Show Cause and Affidavit. By filing this document with the court, a court hearing will be scheduled before a Family Court Commissioner and the parties can present their case for the modification.
