Word documents can be filled in online and then printed.  PDF forms require Adobe Acrobat Reader and are not fillable forms.  The PDF forms must be downloaded on your printer as a blank form and hand-printed or typewritten.  Clicking the underlined summary link will give you additional information about the form itself.

Instructions for Completing Pro Se Forms
  1. Complete the appropriate parts of the form including the attachment as it applies to your case. The link for the attachment is below.
  2. Sign the completed forms.
  3. Present the document to the Child Support Agency if you have a case with that office or if anyone is receiving or applying for Public Assistance (Medical Assistance, Child Care, Food Stamps, W-2)
  4. Bring in the document for the Judge's signature.
  5. After the document is signed by the Judge, make three (3) copies.
  6. Bring the original document and the three (3) copies to the Clerk of Courts Office for Filing.
    • The original document is kept by the Clerk of Courts
    • The copies are for:
      • Petitioner
      • Respondent
      • Child Support Agency

When parties are in agreement to modify their existing orders, they can complete and file the Stipulation and Order to Amend Judgment for Support/Maintenance/Custody/Placement with the court. Once approved by the Judge, the order will be modified without the need for a hearing.

If the parties are not able to agree on a modification of Support, Maintenance, Custody and/or Placement, either party can opt to file Notice of Motion and Motion to Change or Order to Show Cause and Affidavit. By filing this document with the court, a court hearing will be scheduled and the parties can present their case for the modification.

If a party is in violation or is not complying with a court order, the following form may be used. This form may be used to order another party to come to court to justify why that person should not be found in remedial contempt of court for past violations of a current order.

If a party feels the custody and/or placement of the court order merits a change and both parties are not in agreement, you may file a Request for Court Ordered Mediation with the Lafayette County Clerk of Courts Office.
