Vital Records

The Register of Deeds office issues certified copies of birth, marriage, death, divorce, and military discharge certificates for those records where the event occurred in Wisconsin.  If the event occurred outside of Wisconsin, your certificate will not be found here.  The Lafayette County Register of Deeds office can issue the following:

  • Birth certificates for all Wisconsin counties. 
  • Marriage certificates for all Wisconsin counties.
  • Death certificates for all Wisconsin counties from September 1, 2013 to present. 
  • Divorce certificates for all Wisconsin counties from January 1, 2016 to present. 

If you are looking for records prior to these dates, you would need to contact the Register of Deeds Office of the County the event occurred in. 

YOU MUST HAVE A DIRECT AND TANGIBLE INTEREST IN THE CERTIFICATE YOU ARE REQUESTING.   
This generally means the immediate family only such as spouse, parent, child, brother, sister or grandparent.

If you are able to come into the office you will receive over-the-counter, same day service.  Please bring the appropriate payment along with an Acceptable Form of ID.   Please see the Fees section for more information.

If requesting your certificate by mail, please provide the following:

  • The corresponding application found under the Forms and Applications section.  Fill in the applicant information (including signature), relationship, number of copies, and the area for whichever vital record you are requesting. One form will be needed for each individual record you are requesting.  
  • A self-addressed stamped envelope.
  • Check or money order.  Click HERE for the appropriate fee.
