Chronic Disease Prevention Program

Blood Test

The Lafayette County Health Department’s Chronic Disease Prevention Program offers an inexpensive alternative for patients to receive blood pressure checks and lab tests to screen for chronic diseases.  This is a very low cost blood pressure screening and lab draw service; therefore Lafayette County Health Department will not bill health insurance companies and will only accept cash or check at the same time of service. All lab draws are by appointment only.

​Who is this program for?

This program is for residents that:

  • Do not have health insurance coverage; or
  • have health insurance plan with a high deductible; or
  • have private health insurance plan that will not cover certain chronic disease lab tests.

Residents with Medicare or Medicaid Insurance are not eligible for this service. 

A Chronic Disease Prevention consult is $75.  The consult is by appointment only and includes the following:

  • Blood work:  CMP (glucose, kidney and liver function, electrolytes, etc.), Lipid Panel (cholesterol, HDL, LDL, triglycerides), and CBC w/diff (RBC's, WBC's, platelets, etc.).  The laboratory tests require a 10 hour fast.
  • Vitals Recorded:  BP, heart rate, weight, etc.
  • Nurse Visit:  Physical assessment and education provided about chronic disease.
  • Referral to a primary care physician or results sent to current primary care provider.

Individual testing is also available.  Pricing per test is as follows:
